HOW TO MAKE A REFERRAL

It’s simple. If you are a senior or know a senior needing help or having difficulty sustaining his or her quality of life, call Catskill Neighbors, 845-280-0459.

A trained volunteer will be assigned to help with issues affecting the senior.
All calls are confidential and we will do our utmost to be of assistance to you.

Policy

Catskill Neighbors’ volunteers may assist a client in the completion of personal forms for an emergency folder to provide basic general information, i.e. name, address, next of kin, primary care doctor, medications, etc. Volunteers do not provide overnight care, nursing care or money management for a client.  If a volunteer believes a client has no support system in place following hospitalization or stay in a rehabilitation facility, they may advise family members of clients to invoke the language “This is not a safe discharge.” If a client needs support services such as: Medicaid, low-income housing, home health aide, or financial assistance, a volunteer can seek advice from their Catskill Neighbors County Facilitator.
